ACC Modification Request e-Form

If you are making any exterior change to your property, you must obtain approval from the Architectural Control Committee (ACC). 

Before completing this form, please review the ACC Guidelines to ensure your project is within the boundaries of what is allowed by the Association, and the ACC Modification Request form for all applicable requirements. 

When inputting description, include measurements, materials, colors, etc. (Example: Exterior Paint - Behr Antique White #23 base, Glidden Stony Creek #2025 accent or Roof - GAF Timberline HDZ Shakewood Algae Resistant High Definition Shingles).

Upload pictures of paint swatches or materials relating to your project in the File Upload fields beneath the description field. 

All required forms are the responsibility of the homeowner to provide to the ACC, not the contractor! This includes (as applicable):

• All Permit Applications (Roof, Building, Electric, Plumbing)
• Contractor License
• Contractor Proof of Insurance (see Vendor Insurance Requirements page on ACC Modification Request form)
• Landscape Plan
• Building Plan
• Site Plan
